Product feedback survey template

This template helps you understand what customers can do, where they get stuck, and which improvement would make the biggest difference. It combines a quick score with open-ended questions that add context.

When to use this template

Use it after customers have had time to try a feature, complete a task, or bring the product into their routine. It also works after a significant change, as long as the survey makes the experience being evaluated clear.

If you want to evaluate the first days of use, the customer onboarding template offers more specific questions.

Ready-to-use questions

  1. How easy was it to complete what you needed to do in the product?
  2. What were you trying to do?
  3. When did you run into the most difficulty?
  4. Which part of the product helps you most in your day-to-day work?
  5. If you could improve one thing right now, what would it be?
  6. Is there anything important we did not ask about? (optional)

How to adapt the survey without leading respondents

Replace “product” with the feature or task being evaluated. The more concrete the context, the easier it is to answer.

Avoid questions like “Did you like the new screen?” They compress too much into one short answer. Ask whether the customer completed the task, then what helped or got in the way.

For other combinations, use the question generator and adapt the result to the moment in the journey.

How to organize the responses

Group comments by task, difficulty, and impact. A repeated request deserves attention, but it is not automatically a priority. Check who asked, the problem behind the suggestion, and how many people face the same situation.

The score shows direction. Open-ended responses help explain why it changed. The guide to NPS, CSAT, and CES helps choose a metric when you need to track the result over time.

The feedback survey guide helps connect each response to the decision your team needs to make.

Common mistakes

  • Asking about a feature the customer has not used yet.
  • Combining product, support, and price in one question.
  • Asking for a long list of ideas before understanding the problem.
  • Treating every suggestion as a delivery commitment.
  • Sending a long survey after a simple task.

Next step

Start with one effort question and two open-ended questions. Then group the patterns and choose which problem deserves a deeper look.
