Customer satisfaction survey template

This template combines an overall satisfaction question with product, support, and experience dimensions. Keep only the dimensions tied to your goal so the survey stays short.

When to use this template

Use this template when you need a broader view of the relationship or a complete experience. For a recent conversation, use the customer service satisfaction template. For a feature or task, start with the product feedback template.

Define which decision the responses should support first. This keeps you from asking about everything and ending up with data your team cannot use.

Ready-to-use questions

  1. From 1 to 5, how satisfied are you with our company overall?
  2. How would you rate the quality of our product or service?
  3. How would you rate the support you received?
  4. How was your experience from start to finish?
  5. What could we improve?

How to adapt the questions

Choose one overall question, one or two important dimensions, and an open text field. A support survey, for example, can keep support and resolution without asking about delivery.

Use the same scale for comparable questions and label both ends. The survey question generator helps adapt the template without inventing unnecessary dimensions.

If the score is your starting point, the CSAT calculator shows the satisfied-response segment.

How to read the responses

Start with the overall question and compare it with each dimension. If overall satisfaction is high but support scores lower than the other areas, that contrast deserves a closer look at the comments.

Group open-ended responses by theme and note how often each situation appears. The guide to customer satisfaction surveys shows how to connect the goal, questions, and analysis.

The satisfaction survey questions can help you decide what belongs in the script.

Common mistakes

  • Asking about dimensions unrelated to the goal.
  • Repeating the same idea with different wording.
  • Using different scales without a reason.
  • Asking for a long explanation after every score.
  • Drawing a conclusion without reading the comment context.

Next step

Keep the questions that support your decision, remove the rest, and read the survey aloud to check that it makes sense. Then publish the link and track scores and themes together.
