Churn Survey Guide: How to Learn Why Customers Leave
A churn survey helps you understand the context behind a cancellation. It does not replace the churn rate; it adds the reasons, language, and moments the number cannot show.
Choose the send moment
Send after the cancellation is confirmed and the person can reflect on the decision. If the conversation is still a negotiation, the answers may describe the negotiation rather than the reason for leaving.
Keep a path open for people who cancel without responding. A missing answer is still a limit to record, and the response rate guide helps keep that denominator visible.
Ask a short, neutral set
Start with one reason question and one optional open field:
- What was the main reason you decided to leave?
- Which part of the experience had the biggest impact on that decision?
- What could have changed the outcome?
- Is there anything else you would like us to understand?
If you use answer choices, include an “Other” option and keep choices distinct. Avoid wording that suggests the “right” reason.

Read reasons with the rate
Compare the stated reasons with customer churn for the same period and population. A frequent reason among a small group can still matter, while a common response from one segment may not explain the whole base.

Separate evidence from assumptions
Group comments by reason, but preserve the original wording. Keep “customers said the setup was hard” separate from “the setup needs a redesign” until the team investigates.
Break patterns down by plan, tenure, product use, and other relevant context. Do not expose personal information in a shared summary.
Turn feedback into a test
Choose one recurring pattern, identify the owner, and define a small retention experiment. Examples include a clearer setup step, a proactive support message, or a change to the cancellation flow. The customer satisfaction template can help compare the experience before the exit.
Record the expected signal and review date. A survey only creates value when the team can see what happened next.

- Sending the survey before cancellation is confirmed.
- Asking for a long explanation when one short field would work.
- Treating one comment as a diagnosis.
- Hiding “other” reasons inside forced choices.
- Reporting the rate without the population and period.
