SaaS Customer Feedback Survey
A SaaS business needs to listen at different moments: when people reach first value, hit a blocker, consider continuing, or cancel. Each moment calls for a different question.
Map the customer journey
- Onboarding: what was clear and where the first steps stalled.
- Activation: whether the customer completed the behavior that matters.
- Product use: how a feature was used and what remains unresolved.
- Support: whether a conversation solved the issue and how much effort it required.
- Renewal: which value needs to keep showing up.
- Churn: what influenced the decision to leave.
The activation survey template starts with first value, while the churn template looks at a confirmed loss.
Keep the moment clear
After activation, ask whether the task was completed. After support, ask whether the case was solved. Before renewal, ask which outcome needs to continue. One general score cannot replace that context.
Add one short open question such as “What most influenced your answer?”. It helps reveal the cause without pushing a list of team assumptions.
Connect the answer to action
Every survey needs an owner and a decision. Activation may belong to product and onboarding; support to customer service; renewal to customer success; churn to product, pricing, or operations.
Write a three-part synthesis: what appeared, who needs to look, and which experiment will be tracked. Without the last step, the survey becomes an archive of opinions.

What to track
Keep the same scale when comparing equivalent rounds. If the goal changes, change the question and record the context. NPS, CSAT, and CES are not interchangeable; the comparison guide explains the difference.
