When to use this template

Send it after customers should have completed their first important task. The moment depends on the product: it might be after creating a project, publishing a survey, inviting someone, or generating the first result.

How to adapt the questions

Define an activation task before sending the survey. “Use the product” is too broad; “publish your first survey” or “invite ten customers” gives respondents something concrete to evaluate.

If the customer did not complete the task, ask where they stopped without suggesting why. Your team may have a hypothesis, but the response should start with the customer's experience.

How to read the responses

Separate customers who completed the task, those who got partway through, and those who have not started. Then group reasons by step: setup, understanding, confidence, access, or lack of priority.

A recurring difficulty deserves a flow, messaging, or support improvement. A feature suggestion becomes a priority only after you understand the task it was meant to solve.

Common mistakes

  • Sending the survey before the customer has used the product.
  • Calling someone “activated” just because they created an account.
  • Asking which feature they want without knowing the original task.
  • Making the survey long when the goal is to find one blocker.
  • Mixing new customers with people who have used the product for months.
