Survey Launch Checklist

Use this survey launch checklist before you publish. A few minutes of review can prevent confusing questions, broken links, and a response set nobody knows how to use.

Goal and audience

  • Is the decision written in one sentence?
  • Is the audience able to evaluate the experience?
  • Is the send moment clear?
  • Does someone own the analysis?

Questions and scale

  • Does every question support the decision?
  • Does each question ask about one thing?
  • Are the answer choices distinct and complete?
  • Are the scale endpoints labeled?
  • Is at least one open field optional rather than required?

Use the question reviewer to catch long, double-barreled, or leading wording.

Flow and experience

  • Does the introduction explain the time and purpose?
  • Does the first screen make the next action obvious?
  • Do required fields behave as expected?
  • Does the progress indicator match the actual flow?
  • Does the success screen explain what happens next?

Invitation and privacy

  • Does the invitation name the experience and channel?
  • Is the response link correct on mobile and desktop?
  • Does the message explain how feedback will be used?
  • Are consent and optional fields clear?
  • Are you collecting only the information you need?

Test before publishing

Complete the survey as a respondent. Test the shortest and longest path, every choice, the back action, refresh behavior, and the final submission. Confirm that the dashboard receives the response you expect.
